uMotif helps you to track and understand your health. Using the app can improve how you and your care team take care of yourself.
You need a registration code to be able to use the app. This will have been sent to you by your health professional.
The simple, beautiful app has been designed in collaboration with patients, Doctors, nurses and health professionals. It can be used by people with a range of health conditions, including diabetes, heart failure, Parkinson’s Disease, arthritis and cancer.
The app helps you to track data relevant to your health, set up medication and task reminders and engage with content selected by your clinician. The data you enter is used to create your unique Health Report.
Your Health Report can help you understand your health better, and enhance the relationships and consultations you have with your clinicians and carers.

Data tracking – using the innovative ‘motif’ tracker
Medication reminders – enter your medications and set reminders so you don’t miss a dose
Task lists – keep track of your regular tasks and get reminders
Daily diary – keep a note of how you’ve been doing and create a photo diary
Content – specific content selected by your clinician
