iPad based spine surgery outcomes assessment tool. This application provides an electronic interface for the patients who are requested to fill out Oswestry Disability Index (ODI), SF-12, and Neck Disability Index (NDI) forms by their treating physicians at various stages of treatment (preoperative, 6, 12, and 24 months after the surgery). The application contains assessment tools in English and Spanish. This application generates PDF files with the patient responses and emails them to the designated email addresses. It also generates CSV files which could be automatically integrated into the patient databases.
