This app is for CKD and dialysis patients to improve the healthcare delivery. To assist the patient in monitoring their improvement and/or progression of their disease through documenting and tracking their blood pressure, GFR, dialysis measurements. Record their medication list, pharmacy, list of physicians and the phone numbers, as well documentation through dates and pictures of vascular intervention.
