Sly Articulation Screening Form-

This app is designed to allow Speech Therapist to screen clients of all ages for articulation errors at the initial, medial, and final position of words.

It assesses /p, b, m, t, d, k, g, f, v, s, z, l, r, th, sh, ch/ and initial blends for /s,l,r/.

The student is given a picture prompt and the therapist records the answer as correct or incorrect.

Just email the scores and additional comments for each assessment after completed. The results page gives a list of all words tested and the response.
