The Seasons Hospice & Palliative Care Mobile App was designed to help clinicians and family members determine if a patient is hospice eligible based on patient diagnosis, co-morbidities, and score from Palliative Performance Scale (PPS).
Once eligibility is determined, an instant e-referral option is made available to the user and a member of our local hospice team will reach out to you within 24 hours upon receipt of the e-referral.
