Clinicians can exchange ideas and insights into the management of chronic conditions. By consolidating resources already available and generating new educational materials tailored to the needs of the medical community, this Web site will facilitate the exchange of ideas and help clinicians practice the art and science of patient-centered care. Primary care physicians, emergency room clinicians, surgeons, nurses, physician assistants, nurse practitioners, hospital pharmacists, and other healthcare providers are all invited to participate in this global community.
