After the application is set up by your health care provider OnTrack2Health:
- Will alert you which medication to take and when
-Provides you secure and direct messaging with your care team, at any time
- Send your health care provider a daily assessment of your wellness
This application will not function without being connected to your health care provider’s OnTrack Care Giver Management Portal. This portal allows your provider to monitor your progress staying OnTrack to Health.
