- A photo of your child with DOB
- Emergency contacts
- A full child profile (e.g. blood type, height, weight, eye colour, hair colour, religion etc)
- Important contacts (e.g. dentist, specialists, poison control, nearest hospital etc)
- Routine (including feeding times, sleep schedule, games played, discipline & ‘house rules’)
- Medical Details
- Custody orders & Protection orders
- Any other Important information; or
