Health Coach is a NexJ Connected Wellness app that helps you to work with a coach and effectively self-manage your diabetes or hypertension. Health Coach allows you to record your health measurements on your mobile device and track progress on your conditions. You are automatically reminded to track your measurements as defined by your coaching program. Together, you and your coach can communicate securely in real-time to address questions and concerns and help fine-tune your program. Health Coach helps you develop a better understanding of the relationship between your behaviors and your biometric measurements – key to sustaining the behavior changes necessary to be as well as you can be.
