Form Wiz is a flexible tool for organizations to collect and review data from its members. It provides a simple interface for the creation of custom forms and the reviewing of responses from organization members. Administrators can create an unlimited number of forms with an unlimited number of questions. Questions may be multiple choice (radio buttons or checkboxes) or free-form (text field).

Upon first opening the app, check the Organization dropdown menu to see if your organization is already in our system. If you see your organization, select it and log in on the next page with the credentials provided to you by your organization's administrator. If you do not see your organization, please click "Create New Account" button to add it to our system. You will automatically be added as the organization's administrator. You must create standard user accounts (non-admins) in order to collect data.

Administrators create/manage forms and users (admins and standard) and can review responses in a sortable data table by clicking the chart icon next to the form name in their interface. Standard user accounts are issued to the organization members from whom data will be collected. When logging in, standard users should click on the form they wish to fill out.
