If you are a patient of CHC just down load the application to your phone and sign in using our secure connection. The app will automatically verify your information. Once you have signed up you will be able to check in for your appointments (1 hour before the time of the appointment), CHC directions, contact information and a list of your upcoming appointments are also available.
