DiabetesManager® is FDA cleared to help people with Type 2 diabetes manage their disease. Designed with the individual in mind, DiabetesManager provides a personalized “virtual coach" that empowers self-management anytime, anywhere through positive behavior change and decision making. DiabetesManager was developed by physicians, diabetes specialists, educators and people with diabetes to help those with diabetes:
• Get real-time support and feedback for making healthy decisions
• Track, record and chart blood glucose levels
• Learn how exercise, carbohydrate consumption and other factors affect diabetes management
• Share diabetes data directly with their healthcare team

Intended Use Of Device:
The WellDoc DiabetesManager® System is a software solution that can be implemented on the web and mobile phone. DiabetesManager functions to securely capture, store, and transmit blood glucose data and information to aid in diabetes self-management, analyze and report blood glucose test results and support medication adherence, and allow for entry of diabetes-related healthcare information and provide educational information. DiabetesManager®-Rx is activated by the actions of a prescribing healthcare provider and provides additional messaging based on real-time blood glucose values and trends. DiabetesManager can be used independently. When DiabetesManager-Rx is activated, DiabetesManager and DiabetesManager-Rx are combined. DiabetesManager-Rx is dependent on DiabetesManager as the foundation and cannot be used independently. DiabetesManager is for use by healthcare providers and their patients (age 21 and older) who have Type 2 diabetes. DiabetesManager is not intended to replace the care provided by a licensed healthcare professional, including prescriptions, diagnosis, or treatment.
