ACO.Connect helps Michigan Pioneer ACO administrators and physicians provide high quality care to their patients by helping administrators and physicians communicate with each other. The app features handy resources and tools to keep physicians informed and connected.

The Michigan Pioneer ACO is here to provide high quality, health care services that improve the health of the population and the communities we serve. For ACO patients, our mission is to improve the health and quality of health care delivered. We will continue to listen to our patients and caregivers and do whatever it takes to help patients through the continuum of the health care system.  For physicians, our mission is to provide them with the resources necessary to aid in the enhancement of patient care.

Accountable Care Organizations (ACOs) are groups of doctors, hospitals, and other health care providers, who come together voluntarily to give coordinated high quality care to the Medicare patients they serve. Coordinated care helps ensure that patients, especially the chronically ill, get the right care at the right time, with the goal of avoiding unnecessary duplication of services and preventing medical errors.
